This guide provides an introduction to SFOs, a summary of requirements for effective reviews and organisational learning from the thematic, characteristics of good-quality SFO reviews, SFO reviewing system improvement example from Wales, practice improvement example from London exploring how they developed a learning culture and an effective approach to learning and staff engagement from the Midlands is discussed.

Based on a thematic inspection of the Serious Further Offences (SFO) investigation and review process (2020).

The report identifies some common issues and provides a self-assessment for areas to use and explores developing inter-agency cooperation in SFO reviews and developing and managing information to victims in SFO reviewing.
