The overall objective of this research and analysis bulletin was to better understand the issues of blame, learning and growth following SFOs in probation, and to outline key lessons and strategies from other sectors in learning from mistakes and tragedies.

This bulletin was prepared by Kevin Ball, Senior Research Officer and Dr Robin Moore, Head of Data Analysis & Research from HM Inspectorate of Probation.

We would like to thank all those who responded to our survey and call for evidence, and took part in interviews and focus groups.
